In this Privacy Policy, the terms “you” and “your” refer to customers, website visitors, and mobile application users. The term “we”, “our” and “us” refer to Wellness Pharmacy, including our head office and individual Wellness pharmacies. “Personal Information” means any information which relates to or is about an individual and allows that person to be identified.
At Wellness Pharmacy, our motto is “Your Health is Our Priority”. We strive to provide comprehensive healthcare services to meet the needs of our patients. We safeguard your Personal Information with care and diligence as set out in this Privacy Policy. The same care and diligence apply to Personal Information we collect online.
By providing your information to us online or otherwise, you consent to us collecting, storing, using, sharing, and disclosing your Personal Information as indicated in this Privacy Policy. We will obtain consent when we want to use Personal Information for a new purpose or for a purpose other than those stated at the time of collection or in this policy. You may withdraw your consent at any time by contacting us using the contact information provided in this Privacy Policy, except in limited circumstances where we are permitted or required by law to continue to use or disclosure your personal information. If you choose not to provide us with certain Personal Information or if you withdraw your consent, we may not be able to offer or provide you programs, products, or services that you requested or could be offered to you.
This Privacy Policy is current as of the “Updated” date which appears at the top of this page. We regularly review our policies and procedures, and we may update this Privacy Policy from time to time. Any changes we make will become effective when the updated version is posted on our website. If you disagree with any of the changes to our Privacy Policy, it is your responsibility to stop participating in our programs and services or purchasing our products. We encourage you to read and understand the latest version of this Privacy Policy.
We do not actively solicit personal or other information from children under 18. If you are under 18 years of age and would like to participate in our programs or purchase our products or services, please ask your parent or guardian for assistance.
We limit the collection of Personal Information to what is reasonably required to fulfill the purposes for which it was collected. We may ask for Personal Information such as your name, birth date, telephone number, email address, and street address. When you visit our pharmacies to fulfil a prescription or to access our other health services, when you participate in our home monitoring services, and when you use our contactless prescription delivery service, we will ask you to provide your prescription and other relevant Personal Information, including personal health information. We may access your personal health information electronically and may collect Personal Information from the prescriber of a prescription or from your other health care providers, and one of our representatives may contact you regarding your prescription. Where you request to have your prescription transferred from another pharmacy, we will contact that pharmacy and collect your prescription and other Personal Information from that pharmacy for the purposes of transferring and fulfilling your prescription. When you use certain smart devices and services that we offer (“Smart Devices”), we will collect your Personal Information such as when you take the doses of prescriptions that have been dispensed in one of our pharmacies using a Smart Blister Pack. These Smart Devices are also be subject to the privacy practices and policies of the third parties that manufacture the devices and offer associated online services.
In registering for an account on our website or mobile application and by using those services, you will be providing us with Personal Information, including your personal health information such as your prescriptions, medical history, and your requested pharmacy pick-up location. We only collect Personal Information if we obtain your consent for its collection and use. How we obtain your consent (i.e. the form we use) will depend on the circumstances, as well as the sensitivity of the information collected. Subject to applicable laws, your consent may be express or implied, depending on the circumstances and the sensitivity of the Personal Information in question. We may collect, use, and disclose your Personal Information without your consent where required or permitted by law.
If you register with our website or mobile applications, including for the purposes of subscribing to newsletters, registering to health education clinics, accessing your medication records, requesting online prescription refills, or viewing your current and past prescriptions, you will receive a username and password when you have submitted your profile information and have completed the account sign up. You are responsible for providing true, accurate and complete information about yourself. You are also responsible for maintaining and updating your Personal Information to keep it accurate, current, and complete. Any person who has access to your account credentials may be able to access any of the Personal Information available in your account.
When you use our contactless prescription delivery service, we will share your Personal Information with the pharmacist who will fulfill your prescription. The pharmacist will use your information in the same manner as if you had attended the pharmacy in person to fulfil your prescription. When you participate in our home monitoring service, we will disclose your Personal Information collected during monitoring visits and otherwise in connection with the service to your doctor. Certain products and services we offer are provided and sold by third parties and those products and services are governed by their respective privacy statements and policies, including Smart Devices and genetic testing services.
Personal Information will be retained in accordance with this Privacy Policy only as long as may be necessary or relevant for the identified purpose of collection, or as may be required or permitted by law, after which time it will be made anonymous or destroyed. We may contract with third party service providers to store, host, and otherwise use and process Personal Information on our behalf (including in Canada or United States). We will do so only if we are satisfied that the third party is a reputable and reliable service provider, and we require that the such service providers keep your Personal Information secure and only use it for limited purposes for which it is provided. By using our services, you consent to the transfer of Personal Information to countries outside your country of residence, which may have different data protection and privacy laws than in your country of residence. While such information is outside of your country of residence, it is subject to the laws of the country in which it is held, and may be subject to disclosure to the governments, courts or law enforcement or regulatory agencies of such other country, pursuant to the laws of such other country. However, our practices regarding your Personal Information will continue to be governed by this Privacy Policy. We take the security of your Personal Information seriously and use a combination of administrative, organizational, physical, and technical safeguards to avoid the loss, misuse, or unauthorized access, disclosure, or modification of your Personal Information. Despite these measures no method of information transmission or storage is 100% secure or error-free, and we cannot guarantee absolute security.
We expect you, from time to time, to supply us with updates to your Personal Information, when required.
You may make a written request to review any Personal Information about you that we have and we will provide you with any such Personal Information to the extent required by law. We reserve the right to charge you a reasonable administrative fee to access your Personal Information. You may also challenge the accuracy or completeness of your Personal Information in our records. If you successfully demonstrate that your Personal Information in our records is inaccurate or incomplete, we will amend the Personal Information as required.
We may require that you provide sufficient identification to fulfill your request to access or correct your Personal Information. Any such identifying information will be used only for this purpose.
